RN Discharge Planner Full Time

 

What You'll Do:

 

We are seeking a dedicated Discharge Planner – RN to join our long-term care team and play a vital role in ensuring safe, smooth, and person-centered transitions from admission through discharge. 

 

Key Responsibilities of an RN Discharge Planner:

 

  • Manage an assigned caseload from admission through discharge.
  • Develops discharge plans and documents interventions and pertinent information in medical records on a timely basis 
  • Conduct psychosocial assessments and develop individualized care and discharge plans 
  • Provide counseling, emotional support, and strong advocacy for residents and families 
  • Collaborate with the interdisciplinary team, participate in care conferences, and facilitate family meetings 
  • Connect residents with community resources and arrange appropriate post-discharge services 
  • Provide crisis intervention and address behavioral or emotional changes 
  • Serve as a resource for staff and families and participate in ongoing professional development

What You'll Bring:

 

Qualifications of a Registered Nurse include:

  • Valid state license as a Registered Nurse (RN)
  • Compassionate and empathetic approach to patient care
  • Interest in the nursing needs of the aged and the chronically ill with the ability to work with both
  • Strong clinical skills and critical thinking abilities
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team environment
