COTA or PT Assistant for Rehab Unit Discharge Planner

What You'll Do:

As a Certified Occupational Therapy Assistant Discharge Planner, help our residents maintain and improve their independence, function and quality of life. Working alongside the Occupational Therapist and care team, you’you will ll provide hands-on support and compassionate care while helping residents achieve their individual goals.

Key Responsibilities of a Certified Occupational Therapy Assistant:

• Provide hands-on therapeutic care by carrying out individualized plans of care under the direction of the Occupational Therapist.
• Support resident goals through therapeutic activities and exercises that promote function, independence and quality of life.
• Monitor and communicate progress, observing resident responses and reporting changes or concerns to the Occupational Therapist and care team.
• Maintain accurate documentation that meets clinical, quality assurance and regulatory standards.
• Collaborate with the care team to provide safe, consistent and compassionate resident care.
• Support a positive resident experience by treating every individual with dignity, respect and compassion.
• Stay current in your profession through continuing education, in-services and professional development.

What You'll Bring:

Qualifications and Skills:

• Current state COTA licensure and CPR certification.
• Minimum of one year of experience preferred.
• Knowledge of basic anatomy, physiology and neurology.
• Ability to communicate effectively and follow written and verbal instructions.
• Strong teamwork, organization and time-management skills.
• Ability to work independently while collaborating with residents, families and care team members.
• Compassion, patience and professionalism when working with older adults and individuals with complex care needs.
• Ability to meet the physical demands of the position, including bending, stooping, kneeling, reaching, lifting and assisting residents with exercises and activities.
• Commitment to resident dignity, confidentiality, Residents’ Bill of Rights and organizational values.
